I, The Participant,  wish to participate in classes, workshops, programs
and/or other activities presented by Traditional Arts Today, Inc., 314 West 9TH Street,
Ferdinand, Indiana, 47532.

PLEASE READ THIS WAIVER AND RELEASE CAREFULLY. BY SIGNING YOU ARE
AGREEING TO THE TERMS OF THIS AGREEMENT AND WAIVING IMPORTANT LEGAL RIGHTS.

In exchange for my, or my ward’s, voluntary participation in the classes, workshops, programs
or other activities of Traditional Arts Today and/or the use of the property, facilities and
services of Traditional Arts Today, Inc., I, The Participant, agree for myself,
or, if applicable, as the legal guardian for, The Minor Participant, to the
following:

AGREEMENT:

I understand and agree that as used here and below that the words “I”, “me”, “my”, “mine” or
“myself” apply to me, as participant or as the legal guardian, and to my ward as participant, as
applicable.

I understand and agree that while Traditional Arts Today makes all reasonable efforts to protect
the health and safety of participants, there are certain risks, both inherent and unforeseen,
associated with participation in these classes, workshops, programs and activities (hereafter
“activities”), and that these risks may include property damage, illness, physical or emotional
injury or death. I understand and acknowledge that it is therefore very important that I follow
all rules, warnings, instructions and directions regarding my participation.

I further agree that I will follow all rules, warnings, instructions and directions (hereafter
“instructions”) given to me by Traditional Arts Today, Inc. or its employees and representatives
(hereafter “staff”), including volunteers, contractors, teachers, instructors or other agents of
Traditional Arts Today. I understand and acknowledge that the instructions regarding materials
handling, tool or equipment use and the need for, and use of, protective safety equipment are
particularly important for my safety and for the safety of other participants and that my failure
to comply with these instructions may endanger me or other participants.

I understand and agree that Traditional Arts Today, Inc. and its staff may at any time, at their
sole discretion, remove me from an activity and discontinue my participation if my actions or
conduct endanger myself or other participants; damage the facilities, premises, tools,
equipment or other property of other participants, Traditional Arts Today or its staff; or if I fail
to comply with any of the reasonable instructions of Traditional Arts Today or its staff.
I understand and agree that if I am removed, I will not receive a refund, and that I may be
denied permission to participate in future activities.

I further agree to pay for any and all damages to the facilities, premises, equipment, tools or
other property of Traditional Arts Today and its staff caused by any negligent, reckless, or willful
act by me.

I understand and agree that while Traditional Arts Today, Inc. takes all reasonable precautions
for the protection and safety of participants, the staff of Traditional Arts Today are not medical
professionals. Knowing this, I consent and authorize the staff of Traditional Arts Today to
provide first aid in the event of injury or illness and, if necessary, to call for emergency medical
services.

Additionally, I consent and agree to be financially responsible for all medical care including
emergency services that I receive arising from, or associated with, my participation in the
activities presented by Traditional Arts Today. Further, I agree to hold harmless and indemnify
Traditional Arts Today, Inc., its board of directors and staff for costs, fees or damages resulting
from the provision of such first aid, medical care, or emergency medical services.

WAIVER and RELEASE:

I understand and agree that the activities presented by Traditional Arts Today are undertaken
at my own risk, with no liability on the part of Traditional Arts Today, Inc. or its staff.
I knowingly and voluntarily assume full responsibility for all risks, inherent or unforeseen,
associated with my participation in the activities presented by Traditional Arts Today and waive
any claim or demand that I may have, now or in the future, arising from my participation in the
activities presented by Traditional Arts Today, including any claim or demand resulting from my
own negligent, reckless, or willful acts, the negligence of Traditional Arts Today, Inc. or its staff
and any other claim or demand that may have been omitted from this Agreement, Waiver and
Release.

Additionally, I knowingly and voluntarily covenant not to sue Traditional Arts Today, Inc., its
board of directors and staff, including employees, volunteers, instructors, contractors and
agents, in both their professional and individual capacities, and their heirs, successors and
assigns for any claim or demand that I may have, now or in the future, whether known or
unknown, anticipated or unanticipated, arising from my participation in the activities presented
by Traditional Arts Today.

Further I release, discharge and hold harmless Traditional Arts Today, Inc., its board of directors
and staff, including employees, volunteers, instructors, contractors and agents, in both their
professional and individual capacities, and their heirs, successors and assigns for any costs or
fees, claims, losses, damages, or other expenses or liabilities, resulting from my participation in
the activities presented by Traditional Arts Today.

OTHER PROVISIONS:

I understand and acknowledge that I may consult with an attorney before signing this
Agreement, Waiver and Release (hereafter “Agreement”), and that my signature below is made
knowingly and voluntarily.

I understand and acknowledge that this Agreement shall be governed by the laws of Indiana,
and that any claim, action, or proceeding arising out of my participation in the activities
presented by Traditional Arts Today shall be heard in a state court of Dubois County, Indiana, or
in the federal court for the Southern District of Indiana.

I understand and acknowledge that should any part of this Agreement be determined to be
invalid, illegal or unenforceable for any reason by any court then that part of this Agreement
shall be severed and the remaining terms of this Agreement shall survive and remain in full
force and effect continuing to be binding and enforceable.

I understand and acknowledge that this Agreement shall be binding upon my estate, my next of
kin, heirs, successors and assigns.

I understand and agree that this Agreement may not be revoked, terminated, modified or
amended without the explicit written consent of Traditional Arts Today, Inc.

I understand and agree that by signing this Agreement I am waiving legal rights which I, my
estate, my next of kin, heirs, successors or assigns may have against Traditional Arts Today, Inc.,
its board of directors or staff.

I affirm that I am at least 18 years of age and that I have read, understood and agree to the
terms of this Agreement.

SIGNATURE OF PARENT OR GUARDIAN IF PARTICIPANT IS LESS THAN 18 YEARS OLD:

I grant permission and consent for the Minor Participant to participate in the
activities presented by Traditional Arts Today in accordance with the terms of this Agreement,
